You can get blood at the Jankalyan Blood Bank only with a completed requisition form signed by the doctor or authorized medical personnel. This form is available at most hospitals, at Jankalyan Blood Bank reception and on the website. You can download, print and give it to the concerned person. Requisition form has to be filled carefully and completely. It is also recommended to sign the consent form along with the requisition form which is useful for Hospital records.
 
Make sure patient’s name, Blood group (if known only), the hospital or nursing home’s name is entered correctly on the requisition form.
 
The patient’s representative should bring the patient’s blood sample along with the requisition form. Courier service also can be provided by blood bank to collect the sample without any extra charges.
 
You can check availability of the particular blood product over the phone. (Mob 8378958690) Blood grouping of patient and cross matching is done with the available unit, for the process of which it might take minimum one and half hours.
 
Service Charges need to be paid for the blood/ blood products.
 
NO replacement donation is required. However, if your health permits, you are welcome to donate your blood to benefit another patient. This can be done on the same visit or at any future date.
 
Each bag of blood carries a label, which gives information on the type of blood product, blood group, date of colledtion and date of expiry.
 
The blood/ blood products should be transported in a cold container. This could be a insulated bucket or bag packed with ice. Only platelets should be transported at room temperature.
 
Please note, Blood should be kept only in special refrigerators. Do not keep blood bag in domestic refrigerator. Issued blood, blood components must be transfused to patient within four hours.
